Community Nursing Care
This community nursing care supports to participants who require higher levels of care and assistance. Our nursing staff will assist with bowel care, wound care, catheter care, ventilation support, subcutaneous injections, PEG and NG Tube feeding and the administration of medication. Usually involving advanced care or training supplied by an EN or RN.
Assistance with daily personal activities
This service assists participants to ensure their personal care and hygiene is always upheld. Participants may be required to complete personal care tasks such as showering, toileting, changing, basic manual handling for on-off bed mobility, etc, this is done in the participant's home.
Development of life skills
This assists participants to gain services that assist them to develop and maintain important life skills such as learning about public transport, resumes, taxes, saving skills, social skills, physical activity skills etc.
Home modifications are changes to the structure, layout or fittings of a participant’s home so they safely access it and move around comfortably in areas they frequently use. This service also includes the installation of CCTV, Smart Alarms, Video Intercoms, wireless Internet/networking & Smart Aircon upgrade
Participate in the community
This allows us to assist participants to attend area’s in their community. This includes taking them on shopping trips, any appointments, leisure activities etc.
Innovative community participation
This assists participants to access and enjoy activities that they enjoy. This assists participants in increasing social interactions and participating in activities or events they enjoy.
Assist with life stage transition
This service aims to assist a participants capacity to transition from one stage to another stage in life, including partaking in major milestones. Providers may be required to provide peer support, mentoring, mental health recovery, coaching etc.
Assistance with household
This service assists participants to maintain their general area of living. Participants may be responsible for cleaning, gardening, basic indoor & outdoor home maintenance, laundry or linen and also meal preparation.
Assistance with travel and transport
This service allows providers to safety transport participants to and from destinations.
